Home telehealth monitoring, with a rapidly developing family of technologies, puts more patient data in the hands of clinicians. This enables more effective management of chronically ill patients at home, optimizes case managers’ time and, by improving the quality of home care, could play a role in reducing readmissions with small, smart, wireless devices that are as reliable as hospital monitors.

Of course, at-home monitoring technology isn’t for every patient with one or more chronic conditions. “Telehealth is a good approach for a segment of our population,” says Eric Rackow, MD, CEO of geriatric care firm SeniorBridge in New York. “But first you need an assessment, in person or over the phone. Are they able to communicate using new technologies? What’s their emotional reaction to interacting with electronic devices?”
