With more than 25 years of clinical and executive experience in the health care industry, Catherine combines her background in nursing and public health with her long history of accomplishments working with managed care organizations to provide leadership and direction for SeniorBridge's Care Management programs. Her 13 year tenure as consultant, manager and director within the health care practice of PricewaterhouseCoopers provided her with myriad opportunities to successfully lead complex medical management consulting engagements for multiple health care organizations including the Department of Veterans Affairs, Empire Blue Cross Blue Shield of New York, Memorial Sloan-Kettering Cancer Center, Humana, Kaiser of the Northeast and GHI. The extensive reach of her career includes work as Deputy Executive Director for NYCLIX, Inc., a Regional Health Information Organization in New York City, where her position encompassed duties as Privacy Officer, overseeing operations and w ebsite content development. Among Catherine's career highlights, she has served as Regional Director of Clinical Quality Improvement for HealthNet of the Northeast and as Director of Clinical Quality Improvement and Clinical Innovations for Horizon Blue Cross Blue Shield of New Jersey. Catherine earned her Master's in Public Health with honors focusing on health policy and administration from Columbia University. She also holds a Bachelor of Science in Health Service Administration, magna cum laude, from the City University of New York and a New York State license as a Registered Nurse.

Pat has been working with geriatric clients for over 16 years in both long-term custodial and skilled settings. Prior to joining SeniorBridge, she worked as a clinical operations manager for a multi-state physicians practice group that provides in-home physician visits to the high risk chronically ill patients of large managed care organizations. Before this, she worked in Long Term Care, where she was responsible for hiring and training staff, directed multidisciplinary care plan conferences with various medical modalities, families, and physicians, and developed and implemented policies and procedures essential to providing quality patient care. She has a certificate in Business/Personnel Administration from the University of Maryland, and received a BS in Health Care Administration from Ashwood University.

Donna R. Baldwin, RN, CRRN, EMT B
Care Manager

Donna has more than a decade experience in the healthcare field and her commitment to excellence encompasses all areas of her work with an emphasis on patient care, advocacy and medication compliance. Her expertise with geriatrics and her passion for empowering clients with education and planning are an asset to her role at SeniorBridge. Donna began her nursing career at Nursefinders in Albany, New York and has worked at a variety of hospitals and rehabilitation centers throughout the New York area and in Massachusetts and Vermont. Prior to joining SeniorBridge, Donna served as Assistant Director of Nursing at Sweetbrook Transitional Care and Living Center in Williamstown, Massachusetts. Throughout her career, Donna has provided strategic direction, coordination and leadership with nursing staff and managed compliance with state and federal regulations in long-term care settings. Donna received a diploma for Graduate Practical Nurse from Questar III, an Associates in Applied Sciences and Nursing degree from Hudson Valley Community College, and Emergency Medical Technician at the Basic Level from Rensselaer County Ambulance Association all in Troy, New York. Donna holds licensure in the states of New York, Vermont and Massachusetts. She holds certifications as a New York State Emergency Medical Technician- Basic, Rehabilitation Registration Nurse, and American Heart Association Basic Life Support (BLS) and CPR instructor. Donna is professionally affiliated with the Association of Rehabilitation Nurses, Vermont Nursing Association and American Heart Association. She is also an active volunteer in the community for the Medical Reserve Corp of Rensselaer County, Petersburgh Volunteer Ambulance, and for the Grafton Rescue Squad.

Diana Miller, RN, FNP
Nurse Practitioner

Diana has more than 12 years nursing experience and a track record of assessing and addressing the needs of her clients in their homes. Prior to joining SeniorBridge, she served as Community Health Nurse and Family Nurse Practitioner at Schenectady County Public Health, coordinating home care Quality Improvement (QI) activities, collaborating with doctors and professionals to advocate optimal quality of care, and providing health education to patients and their families. Her deep nursing expertise includes also includes women's health and substance dependence. Among her accolades, Diana has been recognized for the M. Grace Jorgensen Nursing Achievement and is a leader in her professional community as a member of the American Nurses Association, Nurse Practitioner Association of New York State, Wound Ostomy Continence Nursing (WOCN) and others. Diana holds licensure in the state of New York as a Registered Nurse and holds certifications as a NYS Nurse Practitioner specializin g in Family Health, American Heart Association (AHA) Professional Rescuer, Medication Administration Trainer, and for Level 1 Healing Touch. She is also an American Nurses Credentialing Center (ANCC) board certified Family Nurse Practitioner. Diana earned a Bachelor of Science in Nursing degree from Russell Sage College in Troy, New York. Diana is professionally affiliated with the Nurse Practitioner Association of New York State and is an active volunteer in the community serving as Treasurer on the Stillwater Elementary School PTA, among being involved with a number of other organizations.

Kira Pezzano, MCSW
Care Manager

Kira is an experienced social work care manager with a passion for helping older people and their families navigate their care. Prior to joining SeniorBridge, Kira helped older adults coordinate home care, behavioral health, advanced directives and assisted living as an Emergency Room Case Manager at St. Peter's Hospital. She has particular expertise with behavioral health, addiction services and advance directives. In addition to her healthcare background, throughout her career Kira has also collaborated closely with legal organizations. She has a diverse background of social welfare experience including providing social work advocacy services for critically under-served youth at Lawyers for Children in New York City and co-leading a creative aging group with the elderly and mentoring children in the after-school program at the YM & YWHA in Washington Heights, New York. Kira earned a bachelor degree in Psychology from Siena College in Loudonville, New York and a Masters in Clinical Social Work from Fordham University in New York City, specializing in Social Work and the Law.
